Summer Camp Registration

Summer Camp Registration can be completed one of two ways.

Medical Information Form (Required)

All campers (both youth and adult) are required to complete a health form in order to attend camp. Please notify us in advance if you will require special accommodations to attend camp. Individual arrangements can be made as needed.

Medical Information Form can be submitted in one of two ways:

  • You can complete the Medical Information Form in your online account after you register for camp. This will save the information for next year as well so all you have to do is update it!
  • Print the Medical Information Form, complete, sign, and bring with you to the first day of camp. (Important: If you have a food allergy or other concern we should be aware of before camp starts, please mail your form directly to camp at least 2 weeks prior to the start date of your event.)

ADULT Medical Information Form (Required)

Adult Campers (18 and over) attending a camp may complete this condensed health form.

An ADULT Medical Information Form can be submitted in one of two ways:

  • You can complete the ADULT Medical Information Form in your online account after you register for camp. This will save the information for next year as well so all you have to do is update it!
  • Print the ADULT Medical Information Form, complete, sign, and bring with you to the first day of camp. (Important: If you have a food allergy or other concern we should be aware of before camp starts, please mail your form directly to camp at least 2 weeks prior to the start date of your event.)

Medication

Medication is any substance a person takes to maintain and/or improve their health. This includes vitamins and natural remedies. All medications are collected, stored, and distributed by camp health care personnel. Please list ALL medications through your online account Dashboard after you register for camp. (Include prescription, over-the-counter, or non-Prescription drugs taken routinely.) Bring only enough medications to last the entire time at camp. Keep it in the original packaging/bottle that identifies the prescribing physician (if a prescription drug), the name of the medication, the dosage, and the frequency of administration.

If you prefer to list medications on a paper form, please see the following instructions:

  • For  adult campers, print the Adult Medication List. Complete, sign, and bring it with you to the first day of camp, or return the completed form to the Camp Minnesota office at least 2 weeks prior to the start date of your camp.
  • For campers under age 18, you can list your current medications on page 2 of the Medical Information Form. If you need more space to complete your list, please print the Additional Page for Camper Medication. Complete, sign, and bring it with you to the first day of camp.

Release of Liability Waiver (Required)

All camps require a signed Release of Liability Waiver for participation in any Camp Minnesota event.

Campership Application

The Camp & Retreat Board feels strongly that everyone who wants to attend camp can do so, and that no camper will be turned away because they can't afford to pay. We provide campership funds to the individuals who need them most, based on individual and family needs. Our expectation is that your local church, personal funds, other sources of support and conference support will combine to cover the registration cost for one conference camping event of your choice. Please allow enough time for this application to be reviewed prior to your camp experience.

A Campership application can be submitted in two ways.

Volunteer Camp Counselor 

All persons interested in serving as a camp leader/counselor in the Minnesota Annual Conference of the United Methodist Church Camping program need to complete a volunteer application form and an a background check.  Simply register online for the camp you will be assisting with.  Once registered, the volunteer application will be available in the Forms section of your online registration Dashboard.  This application should be completed a minimum of two weeks before camp to allow time to complete background checks.  Thank you for serving in this ministry!
